  • Print and complete the Doctor Appointment List and prioritize your concerns

  • Take information with you to discuss with your Provider

  • Print and complete the Medication Worksheet to discuss at your appointment

  • Consider bringing a family member or friend to your appointment

  • Keep your Provider up-to-date

  • Be sure you can see and hear as well as possible

  • Request an interpreter if you need one
